Farmington Family Practice is now offering a Direct Primary Care subscription option FFPplus!
Over the last several years we have seen many changes to the healthcare landscape. As many of you are very aware, health insurance premiums are increasing while benefits are decreasing. Consequently, many of our patients have asked if we would be willing to offer a subscription payment model at Farmington Family Practice.
Our clinic is excited to announce a new Direct Primary Care subscription option, FFPplus. FFPplus is a membership service that includes unlimited care at Farmington Family Practice for a monthly membership fee. Please see the benefit and fee schedule as listed below. This membership service is an option and not required and this is not a health insurance plan.
Our FFPplus Membership Fees are as follows.
$80 per adult Under the age of 50
$105 per adult age 50 years to 64 years
$10 per child (Under 18) *$30 max charge for children per family, families with more than 3 children will not be charged for the additional children
$55 per child without an adult enrolled $165 max charge for children per family, families with more than 3 children will not be charged for the additional children
Membership Benefits
Unlimited office visits with your provider as currently scheduled with no copays or additional fees
All in-house lab and xray services are included (strep test, flu test, CBC, A1C, lipid panel, Chest Xray, etc, a full list can be provided upon request)
All pediatric vaccines included (adult vaccines are available but at additional cost or they can be administered at your pharmacy or the health department)
Access to Farmington Family Pharmacy with standard discount pricing
Telemedicine option. This will be exclusive to FFPplus members. This will allow telephone or video encounters with your provider from home or other locations as appropriate.
